Central Bedfordshire Council Tax Reduction Forms: Complete List and Where to Find Them

The Forms You Might Need

Central Bedfordshire Council uses specific PDF forms for council tax support claims that go beyond the standard online application. Not everyone needs all of them — which forms apply depends on your employment status, who is applying, and whether you have children in paid childcare.

Here is the complete set, with when each one applies.

Proof of Earnings Form

Who needs it: Employed applicants who cannot provide the required payslips. CBC lists five consecutive weekly payslips, three consecutive fortnightly payslips, two consecutive monthly payslips, or an official Proof of Earnings Form completed by the employer.

When to use it: If you have just started a job and do not yet have enough payslips, ask CBC whether it needs the employer-completed form.

Director Proof of Earnings Form

Company directors may need CBC's separate Director Proof of Earnings form. Check the council's instructions for the evidence required for your circumstances.

Proof of Self-Employed Income Form

Who needs it: Anyone who is self-employed. The form details trading income and expenses so the council can assess net trading profit, which is used to determine your income band.

When to use it: If you do not have formal audited accounts. The council may also request supporting bank statements to verify the figures.

Appointee Form

Who needs it: Anyone applying on behalf of someone who lacks the capacity to manage their own financial affairs — typically a relative, carer, or professional advocate acting for an elderly, disabled, or mentally incapacitated person.

When to use it: Helping someone fill in the form does not require appointee status. But if you need legal administrative authority over their council tax support claim — receiving correspondence, signing documents, reporting changes — the appointee form is mandatory. Once submitted, you become legally responsible for the accuracy of the claim.

Proof of Childcare Payments Form

Who needs it: Claimants who want qualifying childcare payments to registered childcare providers disregarded from their countable income.

When to use it: CBC uses a joint Proof of Childcare Payments Form to verify childcare payments with the provider.

Backdating Request Form

Who needs it: Anyone requesting that their council tax support start from an earlier date than the application date. Working-age residents can request up to 12 months of backdating, but must prove continuous good cause for the delay.

When to use it: Submit this alongside medical certificates, hospital records, or other evidence explaining why you could not apply sooner.

How to Get and Submit the Forms

All forms are available as PDF downloads from the CBC benefits forms page at centralbedfordshire.gov.uk. Submit completed forms by email to [email protected] (quote your claim reference number, which starts with 900) or upload them through your My Account portal.

The Central Bedfordshire Council Tax Reduction Guide explains which forms apply to each situation and walks through the evidence requirements for each one.
